Adults - $450 yearly
Children (under 15) $350 yearly
- 2 regular exams
- 2 regular cleanings
- 1 set of bitewing x-rays (unless you are due for FMX)
- fluoride (if requested/recommended)
New Patients: includes FMX (full series x-rays) and 1 Comprehensive exam ($147 value)
Periodontal Membership Plan
$750 yearly
- 2 exams (1 comprehensive)
- All necessary x-rays
- 3 periodontal cleanings (if a fourth cleaning is necessary, an additional $150 fee)
Plans include 15% discount on all other services offered in our practice.
- No yearly Maximums
- No Deductibles
- No waiting periods or denials
Payments for plans are due at the time of registration and are non-refundable.
